Diabetic Footwear Requirements
Clinical foot evaluation by your primary care physician, or endocrinologist.
Doctor’s documentation of one or more risk factor(s) that qualifies you for the Therapeutic Shoe Program. Request a copy of the office notes from your visit with your doctor from the date that your paperwork was signed. The office notes must state that the doctor performed a foot exam.
Detailed written script for therapeutic footwear (diabetic shoes)including:
Number of pairs of inserts,
Signed by physician
Same date as office notes
Statement of certifying physician form
Signed by physician (cannot be signed by LPN, RN, FNP, NP, PA).
Dated the same date as the office notes.
All paperwork is only valid for 90 days from the date signed until you receive your shoes.
Ask the doctor to fax the signed paperwork and office notes to Abingdon Therapy Services at (276) 628-7543.
When we receive all paperwork from your doctor, with the correct information, and proper signatures and dates, we will call you to set up an appointment to be fitted for your shoes.
Please understand that we cannot fit you for your shoes and inserts before we have the proper signed paperwork and correct office notes for our records.
